Disease-Specific Provincial Surveillance Forms
- The data elements collected on disease-specific provincial surveillance forms include the data elements collected for all communicable diseases, those collected for vaccine preventable diseases (if appropriate), as well as any disease-specific variables required. Together these form the minimum dataset for each disease. Those variables required for provincial reporting are noted on the provincial surveillance forms with an asterisk (*).
